If a physician believes that a patient needs care that is above and beyond that included in the guidelines, the doctor take certain steps to obtain authorization for the procedure, test or treatment. It’s critical that your doctor knows the correct forms to submit, and knows what needs to be included with the request for treatment, so you have the best chance of a full recovery.
